Dog Name *

Breed - Include your dog’s full breed or known mix. For example, list a Labrador-Great Pyrenees mix rather than simply “Lab” , as coat thickness, grooming needs and appointment length will differ considerably. If not sure of the breed mix, please write that here. *

Behavior & Handling: Disclose any history of biting people or other animals, along with known fears, sensitivities or difficulties during grooming. This helps us plan for everyone’s safety and your dog’s comfort. *
